Applications for participation in the Interdistrict Open Enrollment Program may be obtained at the Superintendent's office of the Benjamin Logan Local School District, 4740 CR 26, Bellefontaine, Oh 43311, or by calling 937-593-9211 x 1003.
A separate application shall be submitted for each student. Applications shall be submitted to the Office of the Superintendent, Benjamin Logan Local School District with the timeline below. Applications will be marked with the date and time received in the Superintendent's office.
Proof of residence (such as a utility bill or similar proof of residence) must be submitted with the application if you are applying for open enrollment for the first time or if your address has changed since your last application.
If you are applying for the first time, a copy of attendance records is required. For high school students, a current transcript or report card is required.
Time Line: The last day to submit applications is May 1, 2013. Note: Timelines may change from year to year. If the application has not been received before the established deadline, the application may be denied. If capacities have not been met this deadline may be extended by the Superintendent.
Applicants will be notified of acceptance or denial the week of August 1, 2013.
