Elementary Information
Welcome to Benjamin Logan Elementary! This is going to be a great year. We will grow and learn together in many ways. Below is important information about the 2011-2012 school year.
The Back to School Nights for parents will be held on Monday, August 29 at 7:00 p.m. for 1st and 2nd grades and Tuesday, August 30 for 3rd and 4th grades, again at 7:00 p.m. Information concerning your child’s classroom will be discussed these evenings, so it is very important that you attend.
Everyone entering the elementary is requested to register in the office and wear a “visitor” badge. This is for the safety and security of all of our students.
We must hear from you if your child is absent from school. Please call our absenteeism phone line at 592-4838, option 2 by 9:00 a.m. Leave a message as to who is calling, the child’s name, grade, and homeroom, the reason for the absence, and if you need work sent home with a sibling or friend.
Any changes in dismissal procedures must be in writing to the teacher. Bus change requests require one week advance notice. Anyone taking/bringing a student during the school day must sign him/her in/out at the office.
If it is necessary for your child to take any medication at school, we must have a request form completed by both the doctor and parent. One form is included with the packet sent home the first day of school. Please take it with you if you need to take your child to the doctor. Additional forms are available in the school office. Forms for medication taken continually must be submitted each school year.
Any student who has an allergy to a specific food should have a medical alert statement completed by a physician and parent. These are available in the school office.
Be sure to mark all school supplies, jackets, lunch boxes, and anything else that may be left behind with your child’s name and homeroom. There is a lost and found box at the office, so ask if something this missing.
School fees are $50.00 for all elementary students for the 2011-2012 school year. Please send a check for each child, separate from lunch money, as they are different accounts. Fees are due within two weeks of the beginning of the school year.
Cafeteria Prices Student Lunch $2.30 Student Milk $ .40
Each student has a cafeteria account and may prepay lunches. We encourage you to pay for at least a week at a time, but you may put any amount in the account. “Extras” may also be purchased with this account, with parent permission. Of course, children may pay each day if you prefer. There will only be two lunch charges allowed at a time. If a child forgets money and already has two unpaid charges, that child will be given a peanut butter sandwich and milk for lunch.
Raider Care is an after school child care program. The cost of this program will be $6.00 per hour this year, and must be pre-paid. The hours are after school from 3:00 until 6:00 p.m. Children must be in kindergarten through sixth grade to attend Raider Care. Snacks, milk and juice are available for the children to purchase for $.40 each. The children can work on homework, or play either in the gym or on the playground. They will be supervised by the Raider Care aide. If you wish to take advantage of this program, please call the Elementary office. Your child’s teacher must have a note in order to have your child stay for Raider Care.
The Benjamin Logan Board of Education requires volunteers and chaperones to complete an Ohio criminal background check conducted by the Bureau of Criminal Identification and Investigation (BCI). BCI background checks need to be completed before you are scheduled to volunteer and/or chaperone. Any volunteer or chaperone that has not lived in Ohio continuously for the past five years also must complete a federal criminal background check through the Federal Bureau of Investigation. Both BCI and FBI criminal background checks are valid for only one year. Background checks may be conducted at the Benjamin Logan Central Office between the hours of 7:30-4:00. The cost is $22.00 for a BCI check and $25.00 for a FBI check. Individuals can pay by cash or check made out to Benjamin Logan Schools.
Because of safety concerns for our children, we are requesting that everyone follow these procedures at the Benjamin Logan Elementary. |
